Policies & Agreements

APPOINTMENTS

If you must cancel or reschedule an appointment, please notify Valerie within 24 hours. If you have missed your appointment or were unable to call within 24 hours to cancel, and the cause was not due to illness or extenuating circumstances, the full cost of the session will be charged.

SESSION LENGTH

Sessions are 55 minutes in length unless longer appointments are requested. If Valerie begins the session late, the full 55-minute appointment will be given regardless of the start time.

FEE RESPONSIBILITY

Full payment is required at the time of service. Fees are listed below. Upon request, the necessary documentation will be provided for you to submit a claim to your insurance company.

EMERGENCIES

If you feel you are in a crisis and need to reach Valerie, please call her after-hours number, 785-1166. Your phone call will be returned within an hour. Counseling fees will apply if the conversation exceeds 10 minutes in length. If you cannot wait for a return phone call, please use the 24-hour Community Help Line: 472-HELP (4357), or call 911 for immediate mental health services.

CONFIDENTIALITY

All communication or correspondence with Valerie is confidential and information will not be be released without your written consent except in the following circumstances:

•Intent to hurt yourself or others is revealed
•Child abuse is suspected
•Communication with a referring physician is needed to ensure continuity of care
•Records are subpoenaed by a court of law
